Ten minutes No card up front Not approved, never charged

Arizona residents 18 and over. Prescription required.

Pickup or delivered GLP-1s and NAD+ are picked up in person in Gilbert, Arizona, at a time you schedule. Hormone therapy is prescribed individually and shipped to your home.
How it works
1 Take the assessment About ten minutes on your phone. Health history, medications, and what you're requesting. Hormone therapy also needs lab work — yours counts if it's within 90 days.
2 A provider reviews it A licensed Arizona provider reviews it and prescribes when it’s appropriate for you — and explains his thinking either way.
3 You get an invoice If approved, we send one invoice covering the product and any labs it required. Nothing is charged before that.
4 We order your product Ordering starts once your invoice is paid.
5 Pickup or delivery GLP-1s and NAD+ are collected from our Gilbert location at a time you schedule. Hormone prescriptions are shipped to your home.
What you pay

One invoice. Nothing hidden.

There is no membership and no monthly fee, and being reviewed costs nothing. If your provider prescribes, you get a single invoice covering your product and any labs it required — lab pricing at our negotiated Sonora Quest rate. You pay once, and only after you know the number.

Your price depends on your plan.

Every product price is published on its page, and GLP-1 pricing never rises as your dose steps up. What varies is which labs you need, so your exact total is confirmed on your invoice before you pay.

Who reviews your request
Dr. Gary Smith
Dr. Gary Smith
MD, MMM, DABFM

Board-certified in family medicine and licensed in Arizona. Every assessment here is read and decided by him personally — not by an algorithm, and not by a rubber stamp.

Safety

What we'll tell you before you pay.

GLP-1 medications, NAD+ and hormone therapy are tools, not miracles. They carry real side effects — nausea, appetite changes, injection-site reactions — and they are not appropriate for everyone.

Honest by design

If Dr. Smith decides treatment isn't right for you, you aren't charged. We'd rather lose the sale than put you on something you shouldn't take.

Ready when you are.

Ten minutes of questions, no card, no sales call. You'll hear back from Dr. Smith in 24 to 72 hours either way.

Weight loss

GLP-1 medications

Weekly injections that quiet appetite and food noise. You request one at the end of the assessment, and Dr. Smith prescribes it if it's appropriate for you.

Pickup in Gilbert
Semaglutide $185

A weekly GLP-1 injection. Most people start here. Doses step up slowly to keep side effects manageable — the price is the same at every dose level.

Tirzepatide $299

A weekly dual-action injection. Often considered when semaglutide stalls or isn't tolerated. Flat pricing across all six dose levels.

Prices shown are per vial, a typical month's supply depending on your dosage. Prices do not change as your dose steps up. Labs, when required, are quoted separately on the same invoice.

About plateaus

They usually show up around month two or three. That's your body adapting, not failing — and it's the sign a dose conversation is due. That conversation is included.

NAD+

NAD+ therapy

For the kind of tired that sleep doesn't fix. NAD+ is a coenzyme your cells use to make energy, and levels fall with age, stress and shift work. Replacing it is studied for energy, mental clarity and recovery.

Pickup in Gilbert
Injectable · 5ml $189
Most requested

100mg/mL. The usual starting point — enough to find your dose without committing to the larger vial.

Injectable · 10ml $259

100mg/mL, double the volume at a lower cost per dose. For people already settled on a protocol.

Nasal spray $159

300mg/mL, 15mL bottle. No needles, and easy to keep in a locker or a go-bag.

Dosing is slow on purpose

Pushed too fast, NAD+ causes flushing, nausea and chest tightness. Your provider starts you low and moves up only as you tolerate it — that is not caution theater, it is how this is meant to be done.

What the evidence says

NAD+ is not FDA-approved for energy, aging or cognition, and the human research is still early. People report real benefit, and we would rather say that plainly than dress it up as settled science.

Process

From assessment to first dose.

Ten minutes on your phone, labs where they're needed, a provider's decision, then pickup or delivery.

1 Assessment

Ten minutes of health history, current medications, and what you've already tried. At the end you tell us which treatment you're requesting.

2 Provider review

A licensed Arizona provider reviews your history and prescribes when it’s appropriate for you, explaining his thinking either way. Hormone therapy requires labs before this step; labs drawn in the last 90 days count. GLP-1s and NAD+ may or may not need labs first.

3 Invoice

If approved, you receive one invoice covering the product and any required labs. No membership, no review fee, no charge before this point.

4 We order it

Your product is ordered once the invoice is paid.

5 Pickup or delivery

GLP-1s and NAD+: we message you when it arrives and you schedule a pickup in Gilbert. Hormone prescriptions are shipped to your home instead.

Who it's for

Adults living in Arizona — a large share of them fire, EMS, and police. Shift schedules are the norm here, not an exception. Treatment is decided by a provider based on your health history.

What it costs

One invoice after approval, covering your product and any labs it required. Your price depends on your plan — dose, medication and labs all vary — so your exact number is quoted on your invoice.

Hormone therapy

Hormone therapy for men and women.

Fatigue, sleep, mood, libido, recovery, body composition. Hormones are individualized — which means labs first, then a prescription written for your numbers, not a catalog item.

Labs required Shipped to your home
For men From $50

Testosterone optimization and the supporting medications that go with it, dosed to your labs and reviewed as your numbers move.

For women From $70

Perimenopause and menopause support — estradiol, progesterone, and low-dose testosterone where appropriate, written individually.

Hormone pricing depends on how many hormones your protocol includes and the form it's prescribed in — a single-hormone cream starts at $70 for a 30-day supply, low-dose testosterone at $50 per vial. Your exact price is quoted on your invoice, with labs included in the same number.

How hormone therapy differs
Labs are required

Every hormone request needs current blood work. If you've had labs drawn in the last 90 days, send them and they count. Otherwise we send an order to Sonora Quest. Lab cost varies by the panel you need, so it's quoted on your invoice — one payment to us, at the negotiated rate under our Sonora Quest contract, with nothing separate to pay the lab.

Individually prescribed

Your provider writes a prescription for your labs and your symptoms. There is no standard dose here.

Shipped to your home

Unlike GLP-1s and NAD+, hormone prescriptions are sent directly to your address — no pickup trip required.

This site cannot be used for urgent or emergency care. Do not wait on a message from us.

Medical direction. RW Balance LLC is a medical practice. All prescriptions are written by Dr. Gary Smith, MD, MMM, DABFM, licensed in Arizona and the practice’s sole prescriber. Nothing on this site is medical advice, and no provider–patient relationship is created by browsing it or by completing the assessment. Treatment is prescribed only when a licensed provider determines it is appropriate for you.

Compounded medications. Some medications we offer are prepared by licensed U.S. compounding pharmacies. Compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness, or manufacturing quality. A prescription is required, and payment does not guarantee one.

NAD+. NAD+ is not FDA-approved for energy, cognition, aging, or recovery, and the human evidence for those uses is still early. Your provider will tell you plainly where it is thin. No outcome is claimed or guaranteed.

Payment. You are invoiced only after a provider approves your request. Lab costs vary by panel and are quoted on that same invoice. Once an invoice is paid, the order is placed and all sales are final — no refunds and no cancellations.

Availability. Services are available to adults 18 and over residing in Arizona. Individual results vary.
